Welcome to my private practice! I believe that transparency in regard to professional and business policy is essential in establishing a strong and trusting therapeutic relationship. My ability to be explicit in this regard, and your willingness to discuss any issues of concern with me, will support the work that we do together. In order for you to be well informed about your rights as a client, and my policies, therapeutic approach, and background, I am providing the following information. Please read this material carefully and ask for any clarification you need before signing the attached form. Although therapy offers no guarantee of positive change, many people do find that it provides the safe space needed to explore, address and alleviate problematic patterns in behavior, mood and thoughts.
Education, training, and experience: I have a master’s degree in art therapy from the Herron School of Art and Design at Indiana University-Purdue University Indianapolis. My post-graduate training includes motivational interviewing classes, Compassion Fatigue provider training, polyvagal theory, and level one Internal Family Systems training. My first 4.5 years after graduating I worked as a therapist and then team lead for a rural community mental health agency. This experience included providing home and community -based therapy for children, working with wrap-around services, and providing therapy to parents involved with the Department of Children services. I also spent a brief period of time providing school-based art therapy as part of a pilot program. My other counseling experiences include working in medical art therapy, specifically in providing support to caregivers at Riley Hospital and adult oncology patients at Community South, both here in Indianapolis.
Starting in 2019 I worked for IUPUI in two different capacities. The first was an adjunct professor teaching Social and Cultural Diversity in Art Therapy, Art Therapy with Adults and Families, and Group Supervision. The second was as the Internship Coordinator during which I provided individual supervision for around eight students a semester who were placed at sites without certified art therapists.
I continue to seek and receive consultation and seek trainings to further my knowledge as a counselor. Currently that consultation focuses on Internal Family Systems. I also participate in a practice group to continue my IFS learning. I also participate in my own therapy and believe this is crucial for me to be able to do my job as a therapist. At the end of this document you will find a list of trainings I have completed in my time as a therapist.

Services provided: I provide art therapy and mental health counseling to adults and adolescents. I use art therapy, psychoeducation, internal family systems, and relational integrative psychotherapy with my clients. Diagnosis: Many insurance plans require diagnoses on file for services to be reimbursed. The longer I work, the more I use and believe in less pathologizing approaches. I like to be transparent that diagnosing is part of the current mental health world structure but that I am open to not using this structure. However, this alternate approach may lead to less accessible mental health care financially. It is an incredibly asinine bind.
Fees: Individual sessions are $150 per 55-minute session. This rate may vary if you are using insurance based on your plan and whether or not I am an in-network provider. If you need to cancel or reschedule an appointment, I request a 24-hour notice to avoid a charge. I generally provide one freebie cancellation each year and may offer more on a case-by-case basis (for example, if you have COVID). I offer four pro bono spots for clients who are experiencing economic hardship at any given time. As a general rule, I am willing to work with clients on payment schedules based on their own payment/HSA deposit schedule. As I often remind clients, insurance takes generally around three weeks to pay me.
Payment: I use an app called IVY that is designed for therapists. It is HIPAA compliant and is easy to use. You will be sent a text message asking you to put a card on file. This can include an HSA/FSA card, debit card, or credit card. After each appointment, I will then charge that card on file unless otherwise discussed.
Online Platform: I use the portal and online platform built into my electronic medical record system, TherapyNotes, for most client communication. It has secure portal messaging that I can use with clients. This is also the platform I use for telehealth sessions when relevant. It is also my electronic medical record system and clients are able to set up a profile to access documentation and request appointments.
Office structure: My office is located within a suite of offices which I share with other therapists. I am, however, an independent practitioner and simply share office space based on mutual professional interests. Our office does have a private waiting room that you are welcome to use. The office is wheel-chair accessible although the ramp is a ways down the sidewalk rather than immediately next to our front door.
Insurance: I currently am in-network with Anthem Blue Cross Blue Shield, United, and most medicaid/Health Indiana Plans. I am happy to provide a superbill for either your insurance if I am out of network, or for your HSA.
Free consultation Session: I offer a free introductory 20-minute phone consultation to see if we may be a good fit and work well together. We can discuss the reasons that bring you to therapy, talk about your goals and I can answer any questions you may have about how I approach therapy.
Appointments: We can schedule our appointments via the client portal, email, or in person at the end of a session. We can also schedule a standing appointment. I generally am willing to work with clients on the frequency of their therapy. In general, I recommend weekly to begin in order to build a trusting and comfortable relationship a bit faster, however this is not required. I am intentional about letting clients know both if I think we need to meet more frequently because of increased need or if I feel comfortable meeting less often due to changes in circumstances.
Ending Therapy: Termination of therapy should not occur by telephone. Completion is an essential part of the process. For this reason, I strongly suggest that we use one to three sessions to conclude our work in therapy. However, if at any time, you find that this therapeutic process is not meeting your needs, you have a right to request a change in direction or discontinue treatment. If more than 60 days have passed since our last contact and I have not received any word from you, I will accept that as your notice that you no longer wish to continue counseling and that our therapeutic relationship is terminated.
Confidentiality: Sessions in my office and via telehealth are held in the strictest confidence and no information can be released about you without your written permission. This confidentiality extends to minors as well. For minors I will give parents a general overview of what we are working on but will not share specifics of sessions.
There are some exceptions when I am required by law to break confidentiality: when a client is posing threat to self or others, or is unable to provide minimal life-sustaining care; when a client reveals a contemplation of a major threat to others; suspected abuse of a child, elder or a mentally disadvantaged person; in response to subpoena from the Secretary of Health. I hope these situations do not arise, but it is my responsibility to care for you when you are not able to care for yourself. If that occurs, I will contact the appropriate authorities or someone on your emergency contact list.
Safety: It is my belief that safety looks different for each client. I work my hardest to treat each client individually as far as what keeps them safe in instances of self-harm or suicidal ideation. I take this very seriously rather than using a one-size-fits all, liability focused approach. What this means is that there may be times we discuss hospitalization, however it is not what I jump to automatically because this may not be what is going to keep a person safe. It is important to me this process is as collaborative as possible. As previously stated, it is my responsibility to care for you when you are not able to care for yourself.
Case consultations: In an effort to provide quality care, I may review your case with a consultant(s) following the guidelines of confidentiality to protect your identity. Consultation with colleagues helps me to provide the best counseling service to you.
Emergencies: As I am often actively in session with clients, if you are in an emergency, I recommend either going to your local emergency room or calling the new mental health hotline 988 or text HOME to 741741. Complaints: If you have a complaint about my professional service, I hope you will speak to me directly so that the problem can be clarified and resolved. However, you have the right to file a complaint with the Indiana Attorney General’s office if you believe you experienced professional misconduct. You may call 317.232.6201.
